General Requirements

1. Must be 18 years or older

2. Pass Central Registry Background Check/SAM check

3. Posses a valid Level 1 Fingerprint Clearance Card

4. Valid Arizona Driver License

5. Clean 39 month Motor Vehicle Report

6. Three verified references

7. Notarized Criminal History Self Disclosure Declaration Statement
